Job Summary: The PSW is responsible for ensuring all support is provided in a safe manner with adherence to all our client’s health and safety policies and procedures including incident reporting, risk management and hazard reporting.

Job Details: Our client is a registered disability support and approved aged care provider servicing the elderly and people with a disability in their own homes.

We are looking for a caring, passionate and highly motivated support person with an engaging personality to join their growing team in Melbourne. Preferred applicants' locations: Point Cook, Altona Meadows, Seabrook, Hoppers Crossing, Truganina, Werribee or Tarneit.

All supports provided by the Personal Support Worker must at all times be carried out in such a way that aligns with the NDIS Code of Conduct, providing independence and peace of mind within the home.

Hourly Rate: $25-$35 - Ideally 7 hours per shift. We would consider part-time for the right applicant.

Key responsibilities will include:
- assisting the client in all areas of personal care
- providing companionship to access community and social engagement
- using positive behaviour support and encouraging independence in choice and decision making
- assisting with walking and transfers
- monitoring any changes in the client's needs and reporting them through progress notes and care plan updates
- observing and recording the limitations/increased assistance needs of the client
- shopping for/with the client
- assisting the client with food preparation and/or feeding (if required)
- ensuring transport to medical appointments, social engagements and other outings
- performing light housekeeping
- encouraging the client to perform tasks as per the care plan
- assisting the client with emotional, psychological, cultural and spiritual needs

To be considered for this position the following are essential:
- Certificate III or IV in Aged Care / Disability / Individual Support and/or equivalent or - greater (or undertaking such certificate)
- Current National Police Check
- Current Working with Children Check
- Current First Aid/CPR Certificate (HLTAID003)
- Current and valid driver’s license and own, fully insured vehicle
- Fluent in English (Any other language would be a plus)
