Job Summary: We are currently looking for a Home Support Assessor within our South Adelaide metro regional assessment service team. This is a permanent full-time opportunity. You will need to be available to work Monday to Friday, between 8:30am and 5:30pm (7.6 hours per day).
Job Details: In this role, you will be responsible for working autonomously in the community to assess and create support plans for My Aged Care clients with entry level needs to help them remain independent in their own homes. You will work with clients aged over 65 (or 50 if Aboriginal or Torres Strait Islander descent) and living in the community to get a better understanding of the clients’ current situation and develop an individualised plan to meet their needs.
