Job Details: About The Role

This is an exciting time for you to be a part of a growing, dynamic and supportive team of professionals providing services that enable our customers to continue living in their homes and the community. This role is responsible for:

Managing and coordinating a portfolio of CHSP customers
Ensuring customer needs are identified so that services can be matched with care worker skills and attributes
Regularly review and monitor customer targets
Laising with the Customer Relationships Team to develop a strong understanding of customer care plans and needs including opportunities for cross referral
Continuously seek to identify opportunities to convert leads into customers
About you - the successful applicant will have:

Relevant tertiary qualifications and/or demonstrated experience in the coordination of aged care packages (or related discipline)
Professional experience in the delivery of community support services within Aged Care
Excellent communication and customer service skills
Demonstrated ability to build and develop strong working relationships and a strong ability work both independently and as part of a team
Strong understanding of the My Aged Care Portal
Strong sales focus
